tonight, pred only half dose, given Cerenia I don’t know if In or not cos fluid drip out. He definitely look flat out, but he can still stand and walk. stop milk thristle totally. I’ll stop his gabapentin, I’ll change to only Buprenophine. 

He is v dehydrated despite daily 50ml Subcut so I’ll up to 100ml. 

pred can’t stop abruptly la, she leave it to decide to stop or not, I’ll change back to sid, so he only unhappy one time per day. 

Glu fru is normal. Diabetic didn’t reoccur due to pred.

I took chem15 mainly bcos of GGT, Cbc of cos to check his current HCT.

Do it, main thing is shrink or suppress the lymphoma aka also to boost red blood cell. But what if body crash leh? It’s gone gone Liao. Though today he is 14%+, but he is fighter who can more or less maintain now, I choose to don’t take the risk. Today dr Brandon given 1.4ml fersabong – dr d give 0.5ml the first time. 

I’ll give darbepoetin, b12 b com tonight. Continue Marbo for one more week. Review in 2 weeks. Dr want check his crea bun sdma – I ok but he said that after poking toto twice, so of cos don’t do it today la. Next round.

219pm: read Totoro u/s report, though it’s Stated lymphoma highly – I still don’t think so. just a lot of things enlarge only mah. Totoro is cute toto, eat well sleep well. Play w sb. Till it’s time to go. 

No comments n etc from clinic. 

Asked dr d whether she want to read his report. 

Dr d:”So kidney and spleen super abnormal. Liver could fit after the diabetes so chronic hepatomata. But suspect tumor/lymfoma kidney spleen. Which with felv fiv is very likely. Lymfoma doesn’t have to be the end. I would consider prednisolon in his case. Will slow down the tumor temporarily and usually make them feel a bit better” 

Re dr nally says she won’t start pred cos of his diabetic history: “Not anymore right? If give low dose, not necessarily get diabetes again. Give low carb food. But there is a risk of getting it again yes” – my conclusion is Safest is do nothing – don’t start pred. Let him be, tumor can’t grow any worse, he is kind of fine now, I just need to earn more money buy Buprenophine for Totoro.

830am: when I enter room and he get up and look at me, I know jialat – cos today he look more alert and want to EAT look. Omg. He alr meow when I serve the rest first. i went in and he realise i m not going to give him food n even remove water bowl. Omg so torturing for him and me. Keep saying sorry to him, told him I bringing along a big can of kakato for him later on, he finish u/s I’ll give him that straight away. But he is still crying now.

12.27pm: Until now still waiting for the u/s – wa Liao she better don’t be late. I highly think she will be late when I want her even a earlier timing. Arghhhhhh 

1247pm: Totoro u/s done, and I open kakato can he gooooo meow and bite n bite n eat. Happy!! 

But a while later dr nally came and say wanna grab him go test for the fluid w the aid of the u/srapher- see is fluid or blood. He bite one big bite of food in his mouth while dr nally carry him away. Totoro cry in the room once. Sigh what happen? actually given a choice to rethink without caring about pple schedule and etc. i won’t think today u/s is necessary. see what dr nally wanna say later on. 

1pm: they can’t get his “fluid” cos too thin? So give up. 

Main findings: kidneys r big, irregular. Spleen irregular. One liver also got issue. Basically summary he is highly got cancer. Need biopsy to confirm which is no way at all. So yap nothing then. I’ll wait for full report. Continue Marbo for how many more days? – 14 days 

taken Cbc – HCT 15.9%, up only 0.8%, at least didn’t worsen n retic is high good. 

u/s here super ex. Same price as echo. that’s y I knew doing this u/s no use. Fip mentioned but I’m sure it’s not. But now I need to monitor his RR. Only worry is the RR n in case he drown.

night: he still touch food as usual, didn’t polish off like last time, but still more than 50% done. 

Got poo again. Both sets of poo no further unicharm. So still got at least 4 pallets in him. 

Whole day did touch food, no further poo today. But somehow 2+pm thought his eyeball start to be yellowish (but eating leh) and at night feel he is weaker. Sayang him n he did eat for me. RC diabetic pouch plus normal tuna w chicken 

9am: did touch food, but maybe 40%.

Poo a lot more than 20cm in total. Dissert his poo 5+6/10 dry, with ard 8 unicharm pallet out 🙂 so left ard 4 more.

Subcut very little 50-80ml, given Marbo and Buprenophine. Gaba n ursofalk I fed directly into mouth nowadays, didn’t add any more med to food. 2 weeks plus didn’t give meloxicam alr, this may be the cause of his kidney issue, last time don’t have issue one. Erm.

She saw the tongue status from 25.8 vs today, still bad but less redness.

She is worried about his high GGT, scare he vomit (not really) and turn yellow (which he didn’t. Recheck Chem15 to see if the ursofalk is working. 

Cbc taken to see the anemic status. 

She felt his both kidneys r small. next step is ultrasound. for today, she recommend 2 X-rays first to see if any tumor can be seen. 

Dr mention “deteriorate very fast” – based on the 25.8 august kidney values. Last time in June his urea is alr up, just that subsequent didn’t test further. 

X-rays – Duno didn’t take well or something bad happen – Dr nally says she didn’t like the xray she wanna do a quick scan for Totoro (ultrasound) – also sound like maybe Totoro had swallow unicharm pallet? Cos she ask me about litter used – X-rays show at least like 12 pallet in his body, can’t even see the organs cos got free fluid ard the stomach, imflammation n infection. later go back room c, Totoro got to be q without pallet, hopefully he puke put or poo out the pallet. 

Rbc 2.72, hct 15.1 – further clash, but be due to the pallet also. But I think he eat pallet due to anemic vice versa la. Today got to jab the darbepoetin already. Today is day 12.

925am: wa all 5 of them except Don come welcome me (the food) at the door. Totoro at the front. Don’t know who is the one who pee all over in room d again. I think I throwing the unicharm bin later on. Don’t need so many in this room anyway. From ip at 436am; is Totoro pee one la, hs shoot out of the bin. Faint.

Didn’t look good, as in quite thin. Mouth sure pain, but not extreme drooling yet. 

Night: i cook selar for them, got eat, in fact today they had 3 meals, each meal i up quantity of food – they seem v hungry. 

Morning: given breakfast tuna w salmon w dehydrated sardine (I’m worrying more cos of lots of bones) – v active in eating, and I serve bistro chicken also come snatch.

Night: eating ok, side by side w alley then both inter whack each other a bit, not aggressive fight. One off n back off type

Night: given tuna w chicken w dehydrated sardine. Got eat very well but im worry about the Sardine bone  

4pm: I should be happy that he make noise cos calling us? He recognise the good vibes cleaner? A bit scary when a cat out of norm so I bring him food – he eat? The hungry Totoro is back. He polished Xiami and daring’s one also. Let him out of room c but their room is a horrible mess. Kibble all over the place 

Night: seem normal.

I serve food and close room c door – let’s maintain the cleanliness  

Night: eyes got discharge again. Erm I don’t think he is stress or anything in q room w the rest, so he must be in pain, mouth? Have but can’t be all. Where is the very greedy Totoro? Something is wrong. Tmr I’m not free. Even on wed 17.8, still got 8 more days to dr d slot, should drag or see a vet on wed first? Sigh. 8 days seem ok… Erm. I did Subcut him tonight plain.

Morning: room open, let him be, but I’ll Subcut him more for now.

Totoro 15.8.22 Mon 

1040am: fr ip: got move about 

morning: all food not touched. Sigh. Given Subcut and Buprenophine. serve new food. By 5+ the food is touched alr. 80% done, not polished like what he normally would. 

Night: poo 5/10 normal, sayang him still respond to me. Duno why merge alr mouth so much more pain meh? Or pancreatitis flare up? Maybe Subcut help, not so much as Buprenophine for mouth. Maybe is pancreatitis again? 

1015am: he didn’t eat, but kibble bowl is messy and the yesterday plate did mess a bit. Today food totally not touched. This isn’t Totoro. Serve extra aixia oldie red. Is he in pain? given a 0.18ml Buprenophine jab. 

1030pm. Happy he cleared both plates means he is in pain? Change is the Buprenophine jab I given in the morning. Serve supper he ate also. Still close door Tmr then open let him roam again. Don and him head hurt a bit 🙂 

Totoro 12.8.22 Fri 

Morning: overnight did touch food, poo only 3 PCs. Open up his door, end up he didn’t eat when door open. Ha Duno them. 

When vacuum uncle come, daring and Kerry run, he follow them run also. So cute 

Night: didn’t eat again, eyes got discharge, look in pain again. Q him in room c again, Tmr must Subcut him and give him buprenophine jab. 

Morning: only keen on food la. I think he did bite on pallet again, a lot of pallet on floor sigh. 

His room is in a mess. 

Night: got 2 patches of vomit. Not v keen on tuna w salmon. Think he quite thin, v hard to Subcut him, fluid keep coming off. Given darbepoetin 

Totoro 9.8.22 Tues 

1pm: I clean up the place and mop the room cos Xiami anyhow pee, Totoro roam out calmly and didn’t cause any issue so I let him be. He like the tunnel? He a little scare of vacuum. 

5pm: fr ip he sleeping in the tunnel 

Night: seem comfy w Xiami. 

Serve food he didn’t eat wor. cons: omg. Pros: know new friend, somehow a bit reserve don’t be so nei to me and beg for food? did luck the aixia oldie red eventually. He even scratched on the round sb 🙂 maybe suddenly got diff toys he played b4 

Totoro 10.8.22 wed 

Noon: didn’t eat leh. Erm … but he did mess up lots of kibble in the room – cos he ate kibble? 

3.16kg – surprisingly still up 30g  

1030pm: even he don’t want to gobble food w his new friends ard. He also don’t need to don’t eat in front of me right? So I bring him back to room c close door, unless I know he did eat on his own if not I got to q him again. 

He poo 5/10, nice form, colour and amount. 

Night: v happy eat a lot. Will hug my leg nai neh … ha – a bit worried about how he can cope w merging w the other 5. 

Totoro 7.8.22 sun 

4pm: he know something is going on. He waited for food but I’m busy removing each of his room stuff away. Become an empty room that he is waiting for unknown. 

5pm: bring him to room c in Q room. He a bit ? – long time no see cats ah? Eat a bit messy in room cos i pour some kibble for him.  

6pm open up room, merge w alley gang. Daring is the first I serve. Totoro dash to eat his plate, daring hiss fiercely and firmly n loudly, Totoro totally no effect, his brain n eyes only got food. Totoro causing harvo (eat here n there, eat until super messy). He even go into room d Duno eat whose share also. I think firmly he gonna be alone but at least can see others la. He shall stay in room c. his share is alr thrice of others. 

840pm: i re-enter for night round, serve him food first n he won’t even bother about everyone alr. 

302pm: fr Ip: got drink water on his own. Formed poo 5/10. food all cleared. 

Status update: since the last pancreatitis flare up, he is eating well now, but muscles just didn’t grow back. I told him I give u so much food u owe me muscles ahhhhh

Morning: wake up so early for his grooming. Scare me he didn’t really touch food b4 going grooming.  I even serve ciao cup he touch a bit. Then off he go – but sigh today not that clean, fur look bad. Sigh.

4+pm; saw him biting litter pallet aggressively, don’t know he got eat or not, so very bad, have to push up his review – most prob I bring to dr d to check mouth also.

1007pm: 11.8. Clean up his cage and serve food. Bistro chicken w kibble. Not keen alr. Added aixia oldie, not keen. Got smell not keen like looking for someone. Physically he isn’t ok. He looks v dehydrate, that’s y I know high iv isn’t that right. he seem to lose weight over these 2 days only. Sigh. 

Sayang him then he start licking the aixia oldie. Weird2 one. 

Down iv rate to 10ml/h, will maintain this for 1-2 days then down to 6ml/h maintenance rate. But problem is: no proper instruction on so Totoro gonna be on iv how long? Fri I’ll bring him for iv cath check, then mon check again if Friday they didn’t change. this bag must change by 12am. 

1206am: change a new fluid bag at 10ml/h, this bag will end on Thurs 226am. Thursday night got to change bag. He looks ok, I’m dead tired. 

7am i auto awake b4 my 7am alarm ring. Check on Ip cam: Totoro like in a squeeze position. Still wake up to check on him physically, alert. Given Buprenophine. he needs it. 

Keep peeing out of cage, rather dilute. But from bloodworks not really kidney issue. Erm … that’s y l want urine to be done.

11Am: I’m back from 5G, he ate maybe only 6%. Got poo 6/10 formed but I press slightly hard. 278ml fluid used. Left: 270ml – discard no use alr. 

1135am: reached n dr see just bring him in, didn’t even tell me what she going to do. Nvm bah. I request sensor to be put on him. 

Glu 17.4 – they use those blood glucose meter. Sigh I’m right that I shouldn’t have listen to the 1:9 ratio. Now reach home 1pm to Tmr 1pm 14h will remain 17ml/h drip rate plain fluid. Then tap down to 12ml/h and decide to become 25% glucose or not depending on the glucose reading. That’s y important I put sensor. 

don’t have weight n etc, but she check his iv cath n rebandage for me. Don’t have consultation time till I forgot to ask about Cerenia jab – Totoro look uncomfy. Poor boy. 

1235pm: rushing back put him on plain drip hopefully he feel better soon. Sigh so long no high glucose alr

1pm: iv rate 17ml, plain fluid start. This bag will end Tues 6.25pm.  Most prob Tues night I can change a new bag.

So tired I want to sleep I so worried about Totoro n yet his painkilller issue is so ridiculously frustrating till I msg Sunny to inform / tell her the whole thing no logic siak. Sigh. Tired I need to sleep.

5pm: I want to do urine analysis – still waiting for them – I want check for

Bladder infection if any. But dr see say she don’t see the need and she say no need ab for now. She is more concerned w the Low glucose, told me to stop insulin totally alr, and today need to give glucose iv 17ml/h. 

End up she say give Clav 0.17ml – didn’t say for how long. Given dose of 0.06ml for Buprenophine and don’t want dispense me – whole thing don’t make sense. Totoro normal dose is alr 0.15ml n more on usual day, why down? Sigh some things can’t finish. I need more loh. have to cut Manja’s for Totoro, don’t know why vet so scared of protocol but simple math should alr account for alr, end up I have to cut another cat who need painkiller cos Totoro need more. 

712pm: 17ml/h iv drip w 55ml glucose added – listen to her this round, cos Tmr I going proper check and put sensor and first night iv cath can’t be that badly done. 3ml flow smoothly alr. This bag will end: Mon 3.22am. will end long cos I will cut the iv rate down during midnight. 

Totoro mess up the Peepad in cage and pee is all over the floor – later I check on him again. 

1036pm: Totoro got eat dinner, pee quite a lot. Clear up. Decide to let him have the high iv drip all the way till Tmr since appt is 1pm, ie means he only getting 17.5h of iv. See Tmr status n decide.

Today he seem tired keep sleeping. Eating time v alert la. 

I wanna log on this for the past 1 week: I had checked 2 main groups of pple who reply me on how long can glargine be used once opened: ans is 28 days, useful ans is max 2 months. sigh totoro first bottle is given on 24.1. Means March 24 by logic is already not effective. I know the logic of not effective but not harmful. Just not effective. But the fact that if REALLY 24march already not effective and he is still doing well, so means he jolly well wean off insulin Liao loh. Isn’t it? So I stick on to this bottle to give the alternate day insulin till his next review where he most prob can stop le. 

Meanwhile since the stop of Marbo I didn’t Subcut him, he looks fine but maybe a little dry. Today try to remember to Subcut him.

• His infection level is always on off one. So for how, not continuing any ab. 
• HCT improve so good la 🙂

Cbc: HCT down to 20, wbc up – dr says she is worried about ? Cancer ? N etc the underlying reason – highly recommend u/s which I told her what’s the diff in doing it? (I can’t get a good answer) so logical me told me don’t do, waste money, money better spent on pain killer n even deco in his room make more sense than taking a typical u/s that might come back non conclusive. 

Start Marbo 0.44ml today for 14 days, dr says no need Subcut daily. 

Vcheck : no more pancreatitis- back to normal 

Lytes all back to normal, I’ll finish the current bag of nacl first, his k is back to 3.5 – dr didn’t tell me to stop but she prefer to give 1.6ml x twice daily orally. Duno how that will taste like. Erm. I think first. 

Back to 猫房 I feel his room got a maggot smell when I clear his cage, still open it up he can choose to lie on bed or – he choose to go into cage, lick aixia but didn’t eat the rest.

1. no free fluid for sure, not in chest, lung anywhere, RR don’t suggest it too. 
2. His spleen is irregular, might be cancerous or anemic (he is today 25%) 
3. His liver is enlarged – might be due to his weight loss this period – fatty liver. 
4. Both kidneys enlarged – >4cm, think he needs to be on iv. 
5. Intestines r fine, no need start id. M/d diet is fine. 
6. Pancreatitis not ruled out (dr ym ask me about vomitting, oh ya toto didn’t show much of this issue) – will take fpl (given dr the v check results) 
7. Dr ym saw the word “triditie” – told her dr see mention if i didn’t treat the current glucose etc issue, will turn that – she say alr show sign of that cos of the enlarged liver.

411pm: dr ym: “did not collect enough blood for spec fpli. The glucose in urine suggestive of diabetes for sure. I’m running a urine culture to be sure no bacterial infection also. But concentration is good so kidney function seems ok. How long has he been on doxy?” 

I also wonder why his kidney seem ok based on concentration yet both kidneys enlarged: “I think perhaps due to the extra hydration. Or simply because his kidneys are getting older. They can compensate and function but maybe under more stress, hence a bit bigger.” 

“Ideally he should start insulin…why are you not keen to start? Have you had a bad experience?” – told dr ym I got zero experience on diabetic cats. 

“I would stop doxy. Even darbo, possibly not really necessary, anaemia likely more due to chronic disease” – thats y I didn’t want to give once weekly also, but to stop is scary for now. 

I mention about glargine, “Yup, I usually use glargine for cats. Twice daily. We can apply a BG monitor and you can check BG at home using the machine. Look up freestyle glucometer. It’s applied directly to the skin and you can scan it to read BG any time. Lasts for 2 weeks.” – I know this one cos of hai Ning gou gou. Check: Sunny don’t have this option so I go hs put iv? N Erm … or sat go aw put iv? Then mon I can check Pcv at Hs also? Need to think of best plan. 

Need to be on iv? – “He is a bit dehydrated so IV could help. Especially because of the pancreatitis, may help recover faster.” 

Since today didn’t manage to take pancreatic fpli alr, So abandon the plan don’t do, straight do iv – “Likely abnormal since diabetes is affecting pancreas”

dr say doxy working on Totoro, review in 3 weeks, these 3 weeks still continue doxy but half click – ok listen to this n book slot 3 weeks later.

1034am: dr see reply :”morning , ok noted . ideally should be on empty stomach for best absorption as per manufacturer instructions. if really need be , might have to give it in between meals with a small snack ( ie not a full meal ). no particular study done to indicate difference in absorption with/without food , so best way is to re-assess his clinical signs to indicate if medication is working”

🐾 retromad1 (start 13.9.21) giving 0.34ml x twice, initially dr see wanna me give 2 more weeks doxy but after knowing I’m starting retromad1 (she sound happy that toto is starting on this) – so stop doxy. n still review in 2 weeks

• toto HCT is 27.2% (just dropped 0.1% but rbc drop) but today she mentioned for me to start this, think she expect his bloods to go up. 

1pm: he accompany me to buy lunch n I bring him back to 猫房二号 – he sit at the door like he wanna something from me. Serve him a plate of cranberry can food – he is eating. Tonight will start retromad1 – oh no the version 6 dosage is v high 0.6-6mg per kg, meaning I start toto on the lowest also need 0.5ml today. – check my version is 5.2 so is 0.35ml. Didn’t even know give successfully or not – but I’m w injuries and Totoro got scolding and whacking and beating from me. Place all meds into his night food w added bistro chicken – but I guess he won’t touch the food. 

Tonight no Subcut no Marbo.

Afternoon: checked: day 02 since he had the dexa jab, he didn’t eat 2 days breakfast which is tuna based without adding med. but night poesie chicken gravy w med he did touch. Erm … but not much. summary; these 2 days after review appetite not good. 

He did vomit some kibble in the cafe box.

Cbc taken, today HCT 26%, just tell me to start iron n see how review again. 

Wbc NEU still high, did came down a little only – continue Marbo for 5 more days. 

Told me to give 125ml fluid instead of 100 – Erm I think he is getting more hydrated due to more fluid and he pee more. I’ll in fact Subcut lesser per day now, Marbo to continue for 7 more days then I review on next Monday make more sense. 

Ok for dr to go ahead to give the dexa jab today. She given IM. just a short cutting 1-2 days to cut down some inflammation.

Pee had been yellow for about one week now. Is it cos I let go of his pet cockroach friend? Tonight I found his friend – haven’t escape from 猫房二号 – n omg I found a colony – told them no choice I got to kill all. All killed non nego. 

His pee colour is really v dark yellow – scary. 

Think toto is lonely he kept a pet –

Sigh. Told that kar Kar tmr got furmigating – either he survive but I want him out of 猫房二号 or somehow if he get poison told him have to find way to die elsewhere cannot harm

Appetite v good. I sometime feel that he is near the door not to see me, but to catch a glimpse on whether can see socks or not. Anyway his appetite is good but he don’t seem to will ever open up to want sayang or what.

I will say toto is a damn smart and Guai cat. I forgot to put Peepads in the pee tray so if he do pee / poo in the tray then it will be a disaster to clean up, I saw him trying to pull the Peepad from the resting area to the bin area – damn smart, I help him put into the bin n he really pee a lot (still good yellow) and formed big poo 5/10.

Evening : serve batang fish and soup – suap finish all. Give him double serving

1am: at bottom deck so like normal – throwing more things in the room, sigh. Totoro drooling make the room smelly- maybe tmr have to drill out the platform so he can’t rest on that anymore  

830am: since the day we move to 4th generation 猫房, no one occupy the upper box in 猫房二号 – today totoro is up there. Duno why his breathing is quite fast. 

10pm: still up there, didn’t eat at all – look the same. Duno why 

12am go in serve extra food he didn’t come down, I put meloxicam and extra aixia to his plate n put at the top – he did eat 

950am: seem to like bistro chicken a lot, will come down to eat extra

Day: Vomits in the room – based on position should be him. Unsure

17 days after last depredil only, haha no use Liao la. Today start meloxicam. Whole day socks keep staying at the top, my plan to put meloxicam in Totoro food must be abandoned – both jingle n socks really can’t touch meloxicam. Have to just give toto direct if I can  – successful Tonight 

Drooling badly, think depredil won’t work anyway, and somemore it’s going to discontinue – should start thinking of giving meloxicam cos food at above most likely eaten by him.

Back in 猫房 at 650pm n went straight to eat kibble.
